Eligibility

Team Members 

Regular full-time team members or part-time team members who have worked an average of 30 hours or more per week from October to October are eligible to enroll in the Total Rewards programs. 

Dependent Verification

If you are enrolling your spouse/domestic partner or dependent for the first time, you will be required to show proof of dependent eligibility. This includes a marriage certificate for the spouse and birth certificate for the child(ren).

To cover your eligible domestic partner, you must complete the Domestic Partner affidavit annually. Please complete the form by December 16 and return it to the Total Rewards team at [email protected].

Dependents

You may enroll your eligible dependents in medical, dental, vision, and dependent life insurance, as well as other voluntary plans. Your eligible dependents include your:

  • Spouse — includes legally married spouse.
  • Domestic Partner — includes an unmarried person of the same or opposite sex with whom the covered team member shares a committed relationship, is jointly responsible for financial obligations, is at least 18, is not related by blood, who has maintained the same residence for a minimum of 6 months, and is not married to or legally separated from anyone else. Please note if adding a domestic partner, benefits may be taxable as applicable by law.
  • Dependent Child — includes biological, adopted, stepchildren or domestic partner children up to age 26. A dependent child cannot be covered by another U.S. Venture team member under any plan, including life insurance.

* If you work part-time (scheduled 20-29 hours per week), you are eligible for medical coverage under the Anthem BCBS High Deductible Health Plan (HDHP – HSA Eligible) only with no company contribution to your HSA (unless you meet the requirements under ACA) and for dental coverage under the Delta Dental Basic Plan only.
